How do I get a health card?
After successfully registering your agency, the following documentation is required to obtain health insurance:
- copy of the MA form from APR*
- ID card (copied/scanned if chipped)
- copy of the decision and PIB confirmation from APR
- health card
- With complete documentation, go to the RFZO office of the municipality of your registered office for insurance verification and card synchronization (unless your residence is in another city, in which case take it to the RFZO office of the municipality of residence).
Don't have a health card?
When getting verification, you also need proof of payment of the fee for issuing the card, on the basis of which the request will be submitted and a certificate issued that you will use until the card is produced.
